This was my first time every being at a rehabilitation center and my experience was excellent! The staff were attentive and caring especially in the physical therapy department. They have a very supportive environment and stay focused on helping residents regain independence and achieve their recovery goal . The facility is very clean and comfortable I socialized with other residents and staff all day! Meals are served on time, and the dining staff are very friendly and attentive. I truly feel I have made some friends for life from encouragements we gave each other and doing daily fun activities that gave me joy! But, most of all the recovery I have made through a positive environment!
If you care about your loved ones then don’t take them to this place. The staff is horrible & is only there for a paycheck. Poop was left on my dad twice overnight. We removed him from the facility due lack of concern & basically staff not doing their job. Reached out to the supervision over the facility & the concern level is the same. Zero stars is what they deserve from me but unfortunately you have to give the bare minimum.
Extremely disappointed with the care and treatment of the elderly residents at this facility. Seniors deserve patience, compassion, dignity, and proper attention, and that was not consistently shown. Very concerning experience for families trusting this rehab center with their loved ones. Horrible! Let me add no security!
Transitional Care Facility on Jason Street I am deeply disappointed with the care my father received at this facility. He was left in feces for 24 hours, which is not only unacceptable but also inhumane. Additionally, he was not fed during his stay, raising serious concerns about the staff’s neglect. To make matters worse, when my mother tried to address these issues, the staff threatened to call the police on her. This treatment is alarming, and I would not recommend this facility to anyone looking for proper care for their loved ones.
If I could give this facility less than one star, I absolutely would. My 96‑year‑old father‑in‑law was admitted from May 21–26 2026, and what he experienced was nothing short of neglect. During his stay, he developed sores on his genitals due to a complete lack of bathing and basic hygiene. He received little to no physical therapy, even though that was one of the main reasons he was admitted. The most disturbing part was that staff left him lying in his own feces long enough to cause chafing and skin irritation. No patient — especially a 96‑year‑old man who cannot fully care for himself — should ever be left in that condition. When my mother‑in‑law begged for help, instead of addressing the situation or showing any compassion, the facility called the police on her. She was advocating for her husband because the staff refused to provide the care he needed. Their response was unprofessional, hostile, and completely unacceptable. This facility failed him in every possible way: hygiene, therapy, communication, compassion, and basic human dignity. I strongly urge families to think twice before placing their loved ones here.
Lovely place with great staff! Did my Clinicals here and the nurses (Devin) who trained me was amazing at his job and with the residents. The administration was so sweet and supportive! Great place with great care.
Houston Transitional Care provides supportive and professional physical therapy team. The staff is attentive and they work closely with patients to support recovery and improve mobility. I give a special credit to the therapists Thi and Comfort for their wonderful encouragement! They motivated me and kept me relaxed all throughout my recovery. Which helped me achieve my goal to a successful recovery. I also really enjoyed the activities offered every day. Chelsey the Activities Director does a great job creating a positive and engaging environment! My main concern was communication with staff at nighttime. There were times when attentiveness could have been better. That, said my overall experience was still positive, and I appreciate the care and effort from the therapy and activities team.
The care technicians were very consistsnt with good skin care iin the genital area and upper body pressure points. Alittle less patient with assisting with meals. After the first meeting on admission The social worker and even administration were very, very difficult to reach. Only once did an administrator return my call. Discharge planning was none exist.

My husband has been here since Jan1st.2026 . First the good things. The Rehab people are nice and so are some nursing and CNA staff. Food is tolerable but its presentation sometimes is less than appealing. Now the concerns: Staff don’t really talk to or engage the patient much just do their jobs quietly and leave. So for non mobile patients that don’t get many social opportunities, the days are long and lonely if family is not there. Although my husband’ s plan states changing briefs and rotating every two hours the reality is 4 to 7 hours with very infrequent rotation and poor blocking. ( I have a camera) My husband is 100% immobile and one of the reasons we choose Houston Transitional was because they told me there where lo-air mattresses on the bed which was true until they took my husband’s off his bed and gave it to someone else a couple of days ago which I am not happy about. My husband has a history of previous pressure wounds so when you take away his lo flow air mattress, not changing and repositioning him every 2 hours he is at high risk for getting pressure sores again. Although I have tried repeatedly to make an apt with the Director of Nursing she does not return my calls. I haven’t yet tried talking to the Director but will try that next. There is a problem with communication between the Rehab department and the Nurses that has affected my husband’s hospital appts greatly. Don’t come here for speech therapy or you will be disappointed. Although I have asked repeatedly for someone to set up my husband for daily teeth brushing it has not happened. I have also asked repeatedly for maintenance to come fix the thermostat in my husband’s room because it is not working and that has not happened. The place is cleanish but rundown and in need of lots of refurbishing. Towels and facecloths are scarce. The only outside sitting area is where the smokers also sit with butts on the ground and in front of the entrance door. What ever company owns this building does not appear to care to keep it up to a higher standard. *Update 3Feb2026, cooling / heating unit replaced in my husbands room with new unit✔️Thank you. ** Updated 23Feb, 2026,1 week+ after move out to different facility – Brief changing and repositioning went from very bad to worst 10 hours no change. -Numerous holes in bed sheets -Significant water coming through room ceiling during rain storm. -Lost of 7 pairs sweat pants and 3 or 4 shirts all labeled. – No return calls from Director of Nursing, Social Worker, Maintance or Business office after five days of repeated calling and leaving messages. ***Beware this facility has a non-responsive administration and does not seem to care about providing services to more involved patients. Go elsewhere. Houston Transitional Care has put up a new 2026 webpage that doesn’t allow customer reviews to be submitted, This should be a warning right there. –

My recovery was successful, and I grew great bonds with staff. Teena motivated me everyday in therapy! I left with a smile on my face and proud of all I accomplished at Houston Transitional Care.
Fantastic physical therapists and occupational therapists. The nurses, CNAs and housekeepers are exceptional. I was very grateful for the outstanding care I received. I especially appreciate the impressive efforts of Chelsey and the gracious attitudes of the entire staff.

I felt very safe at Houston Transitional Care. The food was very good, and I enjoyed the caregivers wonderful care for me every day! The nurses are very attentive and always make sure residents are taken great care of everyday! 🙂

At my stay here at Houston Transitional Care it was very pleasant. With that I would like to acknowledge the Activities Director Chelsey. She is a very energetic and positive Activities Director. Any activity residents like to do she makes happen with fun! On our Bingo days me and other residents get very excited! We look forward to attending! As for crafts I enjoyed feeling creative and loving what we do. She is never rushing us to finish activities and lets us take our time. She talks with us and socializes with all the residents. From my opinion she is a person of understanding and I’m going to truly miss her!

This is an excellent facility the staff and nurses are very nice.The activity director Miss Chelsey is very active with the clients and the activies she is always checking on the patients encouraging them to participate in activities such as bingo,crafts,movies physical activity healthy exercise activities and checking on their well being how their day is going and brightening up their day.

Neglect: Loved one brought in 5pm in the evening. We were never greated with any head of staff or nurse to go over any of my father’s needs. An aid was the only one that got my fatter in his room. (exceptional aid!) We used our own camera and private sitter! Next morning 7:14am I called EMS to take my fatter back to the VA as he was not straight catheter for over 12 hours. Obviously the night shift nurse failed to do their job as my fathers order was that he is ti be straight catheter every 6 hours. When I arrived at the facility that mornings before EMS arrived a new nurse had come on shift told me she did not have an order to straight catheter my dad till 9 am. This was just not acceptable care in my opinion.

Took 30 minutes for an aid to get to my father’s room, after pushing the call button.They obviously didn’t read his chart that said, he had to be straight catheter every 6 hours.He bladder was not relieved from 5:00pm till we had an ambulance get him out of there at 9:00am. They are sweet talking to you when trying to get a love one there, then your ghosted. We had put our own camera in the room thank goodness to know what help he didn’t get.
