Houston Transitional Care is a wonderful nursing and rehabilitation center in southwest Houston. Most of the staff are caring, understanding, and patient-oriented. The building is renovated and beautiful. Being only in my 40s and suffering a horrible injury, coming to a nursing facility after my hospital stay was concerning. However, the staff here- from social work to physical therapy and activities- have made my rehab experience better than I could ever imagine. On shift change, appropriate information is passed. Everyone is up to date and the new nurses who have come onto their fresh shift smile and are inviting and welcoming. The attending physician and accompanying nurse practitioners are there to support and to help, and patient comfort is a top priority. I have been here nine days and I am well on my way to reaching my goal to return home. Although it may take another week or 2 for me to be completely ready, I trust and have faith in the staff here to prepare me to continue on my journey, overcoming my injury and returning to normal life. Some staff that are notable to my stay are: Chelsea in Activities; Stephen, KeeLey, Jeannette, and Hannah, my nurses; Kim, one of the most capable nurse aides; Melda and Mike, in dietary; Mike, Norma, Teena, and T’hi in rehab; and Brandy, Mya, LaShanna, Cynthi, administrative staff. I would recommend this facility time and time again for anyone needing rehab after injury or skilled nursing- whether short or long-term. Great job, HTC! Jacob – rm 168
At Houston Transitional Care I worked very well with all the Physical Therapy! The Therapists and Nurses are very patient and kind! Before I got there I wasn’t able to get out of bed or do any task independently. I am now going home confident that I can do tasks successfully and needing minimum care. I have met many people at Houston transitional Care and some I know I have made friends with forever. The activities Director Chelsey excels at here job and I truly enjoy the bond I have grown with here. And, the motivation she has given me all throughout my stay at Houston Transitional. I recommend this facility to everyone that needs rehab and wants to have a successful recovery.
I’m currently a caregiver for a resident here at Houston Transitional and truly have enjoyed the time we have here. The facility is very clean, the staff friendly and the physical therapy was successful with helping with my clients recovery. I wouldn’t send my dead dog there! They were terrible! They don’t communicate within the departments . They say you’re paid up through a certain date and then say you’re not 🚭 so they are crooks!
The staff here is fantastic! Everyone here goes above and beyond and truly care about their patients. We have been treated so well, everyone who comes into my husband’s room has become like family. I came to Houston Transitional care after I was in a car accident and broke my arm and fractured rib. The recovery was very painful challenging and nothing like I have experienced before. The physical therapy team were very kind, patient and very encouraging that I would be able to do things on my own again. They worked with me at my own paste and helped me become confident! Even days I felt frustrated I still looked forward to going to therapy and the activities planned everyday! ❤️❤️I love the activities I did that helped my recovery and also make new friends. I’m proud of my progress I made in recovery and thankful to everyone at Houston Transitional care who helped me along the way.
